If you are new to our practice or have not been seen in the last three years, you must complete the nine new patient forms listed below, in addition to, the DXA Scan Questionnaire.

If you are a returning DXA patient of Carroll Arthritis, LLC, and only visiting for your scan and are not making a doctor office visit, you need only complete the DXA Scan Questionnaire.

DXA Scan Patients

New DXA patients must also complete the DXA Risk Assessment Questionnaire. Print the questionnaire linked below, fill out and return to us during your next visit.
