Information: Important Patient Information

Thank you for choosing Carroll Arthritis, LLC You have scheduled a rheumatology consultation with us. In order to serve you better, we request a new patient packet to be completed. The necessary forms are available for download. Just bring the completed forms with you at the time of your appointment. Also, please provide us with the following items:

• Picture ID (i.e. driver's license)
• Insurance card(s)
• Co-payment (cash, check, MasterCard, Visa, or Discover)
• Referral (if required by your insurance company)
• Recent blood work, x-ray reports, office notes (your doctors can fax these to our office)
• List of current medications
• Name, address, phone and fax number of the doctor who referred you

Please arrive 15 minutes prior to your appointment time.
